Where questionnaires shine
The same instrument serves very different rooms. Six use cases, and the title each one deserves.
Academic research
Theses, dissertations and journal studies run on questionnaires. Titles here must be formal and precise — they appear in methods sections and citations.
What works: "A Questionnaire on [Topic] for [Purpose]" in the formal tone. Check your institution's conventions and your supervisor's preferences before finalizing; many departments have house styles for instrument titles.
Customer experience
Post-purchase, onboarding and support questionnaires. Titles should be friendly and brief, with the time cost visible — customers decide in the inbox whether to click.
What works: "How Are We Doing? [Topic] Questionnaire" plus "2-minute" near the title. Trigger the send close to the experience while memory is fresh.
Human resources
Engagement, onboarding, exit and pulse questionnaires. The title must signal purpose and confidentiality — employees answer honestly only when they trust the process.
What works: "Your Voice: [Topic] Questionnaire" with an explicit anonymity statement. Never promise anonymity the process cannot keep.
Healthcare and wellness
Intake forms, symptom checkers and wellness assessments. Titles should be calm and non-alarming; the respondent may be anxious before question one.
Handle with care: health questionnaires should be designed or reviewed with qualified professionals. A thoughtful title never compensates for a careless instrument.
Events
Post-event feedback with a short shelf life. Name the event explicitly and send within 48 hours.
What works: "How Was [Event]? Feedback Questionnaire" — the event name does the recognition work, the time promise does the motivation work.
Market research
Concept tests, pricing studies and brand trackers. Titles should be neutral — leading titles ("Don't you love...") produce leading data.
What works: plain descriptive titles and a professional tone. Save persuasion for the marketing copy; keep the research instrument clean.
Matched titles for each case: the QueryCraft generator has dedicated templates for research, customer, employee, academic, health and event questionnaires.
